De vraag is vast al eens eerder gesteld:
Ik ben op zoek naar onderzoeken die direct of indirect het verschil hebben onderzocht tussen de twee verschillende SRS operatietechnieken: penisinversietechniek en de non-penisinversietechniek. Ongeacht de conclusies van het onderzoek wil ik ze graag in beeld hebben voor mijn 'gevecht' met mijn zorgverzekeraar over vergoeding van mijn in het buitenland uitgevoerde SRS.
In het kader van forumfatsoen: ik ben niet op zoek naar meningen over wat wie het beste vind. Ook snap ik dat iedereen wel een geval kent waarin het helemaal is misgegaan. Ik ben op zoek naar onderzoeken. Het Transvisie onderzoek van deze zomer heb ik al. Wie kan me helpen?

Ben ook wel benieuwd of deze studie (https://www.ncbi.nlm.nih.gov/pubmed/27623991) onderscheid maakt tussen penile-inversion en non-penile-inversion.
Ik heb het idee dat die laatste categorie gewoon nog niet onderzocht is.

En in een van de onderzoeken wordt al aangegven dat penile inversion het meest onderzocht is. Dus tjah, wat je zegt is waar.

Wat me aan de onderzoeken opvalt is dat het vooral de genderchirurgen zelf zijn die hun eigen waar aanprijzen. De eerste is van het VUmc, de tweede van het team van Krege en de derde van het UZ Gent...

Het zal je verbazen hoeveel onderzoek zo'n commerciele ondertoon heeft. Vooral in de medische wereld zit er altijd wel een bedrijf achter die er wat mee te winnen heeft. Erg jammer, want je krijgt er soms wel biased resultaten door. Ik hoop dan ook dat de onderzoeken naar gender chirurgie iets minder biased zijn, ondanks dat er bepaalde ziekenhuizen het onderzoek gestart hebben. Dat is trouwens zo gek nog niet, want dat zijn ook de mensen die technieken willen verbeteren.

Re-do vaginoplasty with rectosigmoid colon in male-to-female transsexuals.
Between April 2000 and February 2010, 29 female transsexuals, aged 26 to 59 years (mean 32) underwent rectosigmoid vaginoplasty due to failed vaginal reconstruction. Isolated segment of rectosigmoid was from 8 to 11cm length to avoid excessive mucus production.
Rectosigmoid is harvested with blood supply originating on sigmoidal arteries or/and superior haemorrhoidal vessels.
Preferably, it should be dissected distally first in order to check its mobility and determine the correct site for its proximal dissection.
Stapling devices is used for the colorectal anastomosis as a safest procedure. Creation of perineal cavity for vaginal replacement is done using simultaneous approach through abdomen and perineum.
Very precise dissection must be done to avoid injury of rectum, bladder and urethra. Introital remnants or perineal skin flaps were used for anastomosis with rectosigmoid vagina.
Postoperative dilation was recommended to prevent purse string scarring with subsequent vaginal stenosis in 6 months after surgery.
RESULTS Follow-up ranged from 10 – 128 months (mean 43). Good aesthetic result with satisfactory psychosexual functioning was achieved in 20 cases.
Complications were: neovaginal prolapse (5), introital stenosis (4), excesive mucus production (4), and temporary diversion colitis (2).
CONCLUSIONS Rectosigmoid colon presents a good choice for creation of neovagina in male transgenders after failed vaginoplasty.
Described refinements significantly decrease the number of complications that usually occur in this type of vaginoplasty.

Long-Term Outcomes of Rectosigmoid Neocolporraphy in Male-to-Female Gender Reassignment Surgery.
Introduction:
Our group has previously reported favorable outcomes of rectosigmoid neocolporraphy. Unfortunately, rectosigmoid transfers are still perceived negatively, usually relegated to secondary vaginoplasties. Our group aims to go beyond these misconceptions to provide an objective investigation into the safety and efficacy of rectosigmoid neocolporraphy for primary vaginoplasty in the male-to-female (MTF) patient.
Methods:
A retrospective review was performed on MTF patients who had undergone primary rectosigmoid neocolporraphy with the senior author, Dr. Laub. The technique involves isolating a segment of rectosigmoid colon on its vascular pedicle. The proximal aspect of the segment is closed, and the distal end opens at the perineum. Patient data including demographics, medical history, complications, and the need for revisional surgery were obtained. Direct inquires were conducted to determine patients’ level of satisfaction with appearance, sexual function, and ease of post-operative recovery.
Results:
One-hundred patients were included over the course of 25 years. Overall, the patients were healthy with minimal comorbidities. A small number of complications occurred including: hematoma, infection, painful sexual intercourse, obstruction, and urethral fistula. Excessive muccorrhea occurred in only a small number of our cohort. Overall patient satisfaction with appearance and sexual function (among those who were sexually active) was high.
Conclusions:
Our study is one of the largest and longest reported series of rectosigmoid transfers for primary vaginoplasty. Advantages include long vault length, self-lubrication, a natural appearance, sensibility, and lack of malodor. Disadvantages include strictures or leaks of the intestinal anastomosis, and the need to enter the abdomen, which adds a layer of complexity to the procedure. Rectosigmoid neocolporraphies have many times been recommended for secondary or revisional surgery when other techniques, such as penile inversion, have failed. However, we believe the rectosigmoid transfer is safe and efficacious, and it should be offered to MTF patients for primary vaginoplasty.
